NOTICE TO PATIENTS OF TWIN CITY HOSPITAL:
Trinity Hospital Twin City acquired the assets of Twin City Hospital pursuant to a Sale Order approved by the Bankruptcy Court on May 9, 2011. As part of that transfer, Trinity Hospital Twin City has agreed to maintain and secure all the patient records that were in the possession, custody and control of Twin City Hospital, except for those Twin City Hospital was not required to retain under law (any that Twin City Hospital was authorized to destroy under law). Trinity Hospital Twin City will comply with all the applicable state and federal laws regarding access to patient records and destruction of patient records. Please do not hesitate to contact the Health Information Management Department if you have any questions or wish for a different disposition of your records.
