We now offer a personalized program for ALL adults who have been diagnosed with two or more chronic conditions such as diabetes, high blood pressure, high cholesterol, heart disease, COPD, arthritis, and obesity.

Instead of trying to figure out what medicine to take and when, tracking down test results to get from one doctor to the next, and calling to set up appointments for testing or routine checkups, you can use our program to receive coordinated care. Chronic Disease Management (CDM) is a collaborative program between your family doctor/provider, specialists, testing centers, and hospitals to make sure everyone is working together.

CDM allows you to focus on what is important to you. Our providers are able to spend more time on your care - even when you are not at their offices.
